تفحص کیفی برای تشخیص اختلالات کژکارکردیهای جنسی و تأثیر آن بر کارکرد خانواده در آسیبدیدگان بمباران شیمیایی سردشت
جنگ بخشی جدانشدنی از زندگی اجتماعی آدمها است و همواره در طول تاریخ وجود داشته و صدمات جبرانناپذیری بر جنبههای گوناگون سلامت بدنی، روانی و خانوادگی آدمها وارد کرده است. پژوهش حاضر، بهمنظور تفحص کیفی برای تشخیص اختلالات کژکارکردی های جنسی و تأثیر آن بر کارکرد خانواده در آسیبدیدگان بمباران شیمیایی سردشت انجامگرفته است. پژوهش حاضر یک بررسی کیفی پدیدارشناسی - توصیفی میباشد و جامعه آماری آن همه آسیبدیدگان بمباران شیمیایی سردشت بودند که در تاریخ 7 تیرماه 1366 توسط رژیم بعث عراق موردحملۀ بمباران شیمیایی قرار گرفتند که تعداد جامعه 223 نفر (130 مرد و 93 زن) بودند. نمونه پژوهش 60 نفر، دربرگیرنده 30 مرد و 30 زن بودند که بر پایه اشباع در هنگام انجام پژوهش و بهصورت نمونهگیری در دسترس انتخاب شدند. در آغاز برای غربالگری آسیبدیدگان مصاحبه بالینی صورت گرفت و بعد، از افراد شناساییشده نمونه، مصاحبه نیمه ساختاریافته به عمل آمد. در تحلیل دادهها نیز از الگوی هفت مرحلهای کلایزی بهره گرفته شد. برآیندها نشان میدهد که شکایتهای اختلالات کژکارکردی های جنسی دربرگیرنده اختلال میل جنسی، اختلال برانگیختگی جنسی، اختلال ارگاسم، اختلال بیزاری جنسی و اختلال درد بخش تناسلی در هر دو جنس وجود داشت. همچنین شکایتهای؛ اختلال نعوظ، انزال زودرس و انزال دیررس ویژه مردان و شکایت؛ درد آمیزش ویژه زنان بودند. در ارتباط با شکایتهای کارکرد خانواده؛ شکست در ازدواج، ناخشنودی از روابط زناشویی، نبود خوشنودی از روابط خانوادگی، نبود خوشنودی از نقش والدینی و ارتباط ناسالم با خویشاوندان و همسایگان؛ در طیف گستردهای از آسیبدیدگان گزارششده است. بمباران شیمیایی بهطور گسترده باعث به وجود آمدن مشکلات و اختلالات کژکارکردی های جنسی در آسیبدیدگان شیمیایی شده است و این اختلالات تأثیر سوء و منفی بر کارکرد خانواده آسیبدیدگان داشته است و بنیاد خانواده و کارکرد خانواده را با چالشهای فراوان روبرو کرده است؛ بنابراین مشاورین خانواده میتوانند با برگزاری جلسات و کارگاههای مشاوره خانوادهدرمانی به خانواده و فرد آسیبدیده کمک بکنند.

اثربخشی زوجدرمانی متمرکز بر هیجان بر صمیمیت هیجانی و جنسی زوجهای ناسازگار
زوجدرمانی متمرکز بر هیجان رویکردی کوتاهمدت و ساختاریافته برای درمان و بهبود روابط آشفته است که بر چرخههای منفی تعاملی پایدار، در اثر آسیبپذیری هیجانی عمیق توجه دارد و بر کاهش آشفتگی زوجی از راه مداخله در سطح هیجانی بهویژه پاسخهای هیجانی دلبستگی محور کوشش مینماید. پژوهش حاضر با هدف بررسی اثربخشی زوجدرمانی متمرکز بر هیجان بر صمیمیت هیجانی و جنسی زوجهای ناسازگار انجام گرفت. جامعه آماری پژوهش دربرگیرنده 100 زوج از زوجهای مراجعهکننده به مراکز مشاورۀ شهر اهواز (مهرآوران، ندای زندگی و مرکز تحقیقات علوم رفتاری دانشگاه شهید چمران اهواز) در فصل پاییز 1396 بود. نمونه پژوهش دربرگیرنده 3 زوج (۶ نفر) از زوجهای مراجعهکننده به این مراکز بودند که با توجه به نتایج پرسشنامۀ سازشیافتگی زوجی (اسپانیر، 1976) با خط برش 101 و با روش نمونهگیری هدفمند انتخاب شدند. در این پژوهش از طرح آزمایشی تک موردی از نوع خط پایه چندگانه ناهمزمان استفاده شد. برای سنجش صمیمیت هیجانی و جنسی زوجها از خرده مقیاسهای صمیمیت هیجانی و صمیمیت جنسی، مقیاس ارزیابی شخصی صمیمیت شافر و السون (1981) استفاده شد. پروتکل زوجدرمانی متمرکز بر هیجان در سه مرحله خط پایه، مداخلهی10جلسهای بهصورت هفتگی (نشستهای 90 دقیقهای) و پیگیری ۲ ماهه اجرا گردید. دادهها به روشهای ترسیم دیداری، شاخص تغییر پایا و فرمول درصد بهبودی تجزیهوتحلیل گردید. یافتهها نشان داد که زوجها در مرحله درمان در صمیمیت هیجانی (82/33%) و صمیمیت جنسی (88/34%) بهبود را تجربه کردند؛ بنابراین یافتههای پژوهش روشنگر این است که زوجدرمانی متمرکز بر هیجان با تغییر چرخههای تعاملی منفی بر افزایش صمیمیت هیجانی و جنسی زوجهای موردبررسی تأثیر دارد.

ساخت نسخۀ ایرانی سنجۀ مهارگری زناشویی و آشکار ساختن ویژگیهای روانسنجی آن
مهارگری میتواند تأثیری شایان در رابطۀ ناسازگار خانوادهها مخصوصاً روابط زناشویی داشته باشد. این پژوهش با هدف شناسایی روشهای مهارگری در روابط زناشویی و ساخت سنجه مهارگری زناشویی، تعیین روایی و پایایی آن در زوجهای ایرانی انجام گرفت. پژوهش حاضر از دید هدف، پژوهشی توسعهای و از دید شیوۀ اجرا توصیفی-پیمایشی است. جامعۀ آماری پژوهش همه افراد متأهل شهر مشهد در سال۱۳۹۶را شامل میشد که با بهکارگیری روش نمونهگیری در دسترس انتخاب شدند. تعداد 310 نفر (155F= و 155M=) به سنجۀ 52 سؤالی مهارگری زناشویی پاسخ دادند. بهمنظور تحلیل دادهها از روایی صوری و محتوایی، تحلیل عاملی اکتشافی، تحلیل عاملی تأییدی، همسانی درونی و روایی ملاکی استفاده گردید. دادهها با استفاده از دو نرمافزارSpss-21 و Amos-24 مورد تحلیل قرار گرفت. با انجام تحلیل عاملی اکتشافی و با استفاده از چرخش واریماکس پنج عامل شناسایی شد که مقدار ویژۀ هر عامل بالاتر از یک بود. از 37 گویه باقیمانده، برای تحلیل عاملی تأییدی استفاده شد. مدل ساختاری مهارگری زناشویی با 21 گویه پس از اعمال پیشفرضهای تحلیل عاملی تأییدی و اصلاح مدل با پنج عامل مهارگری هیجانی، مهارگری از طریق بیتوجهی، مهارگری از طریق خشونت کلامی، مهارگری از طریق انزوا و مهارگری از طریق پنهانکاری تائید شد. همچنین روایی همگرای این سنجه با خرده سنجه سلطه-اطاعت شانزده عاملی کتل رابطۀ مثبت معنیدار۰٫۲۴ به دست آمد (۰٫۰۰۱p<). ۰٫۹۳۳=α و مقدار ضریب اسپیرمن-براون و گاتمن به ترتیب ۰٫۸۹۴ و ۰٫۸۸۶ است که نشان میدهد سنجه نهایی دارای روایی همگرا و واگرا است و از پایایی برخوردار است. نتیجه، سنجه مهارگری زناشویی با پنج عامل پایا و روا برای زوجهای ایرانی و پژوهشهای حوزۀ خانواده و زناشویی در جامعۀ ایرانی است.

درمانهای یکپارچه نگر و زوجدرمانی: هم سنجی اثربخشی رویکردهای یکپارچه نگر، زوجدرمانی متمرکز بر هیجان و زوجدرمانی رفتاری یکپارچه نگر بر بخشایشگری زوجهای دچار تعارض زناشویی
زوجدرمانی متمرکز بر هیجان و زوجدرمانی رفتاری یکپارچهنگر از جمله درمانهای یکپارچهنگر هستند که بر چرخههای منفی تعاملی پایدار در اثر آسیبپذیری هیجانی عمیق توجه دارند. پژوهش حاضر با هدف همسنجی اثربخشی زوجدرمانی متمرکز بر هیجان و زوجدرمانی رفتاری یکپارچهنگر بر تسهیل بخشش زوجهای متعارض به انجام رسید. جامعه آماری پژوهش شامل کلیه زوجهای مراجعه کننده به کلینیک روانشناختی مهرآوران شهر اهواز بود. نمونه پژوهش با استفاده از روش نمونهگیری هدفمند انتخاب شدند که شامل 6 زوج(12 نفر) از افراد مراجعه کننده به این کلینیک بودند که با توجه به نمره پرسشنامه سازگاری زناشویی جزء زوجهای پریشان قرار میگرفتند. در این پژوهش از طرح تجربی تکموردی از نوع خط پایه چندگانه ناهمزمان استفاده شد. پروتکل درمانی زوجدرمانی متمرکز بر هیجان و زوجدرمانی رفتاری یکپارچهنگر در دو تا چهار مرحله خط پایه، 16 جلسه مداخله 60 دقیقهای و پیگیری دو ماهه اجرا گردید و آزمودنیها به مقیاس بخشش رنجش خاص زناشویی(پالیری، فینچام و رگیالیا،2009) پاسخ دادند. دادهها به روش ترسیم دیداری، شاخص تغییر پایا و فرمول درصد بهبودی تحلیل شدند. یافتههای پژوهش نشان داد زوجدرمانی متمرکز بر هیجان، نسبت به زوجدرمانی رفتاری یکپارچهنگر در متغیر بخشش کارایی بیشتری داشت. میزان بهبودی در متغیر بخشش در گروه متمرکز بر هیجان در مرحله پس از درمان و پیگیری 14/47 و 94/48 درصد و در گروه رفتاری یکپارچهنگر در مرحله پس از درمان و پیگیری 40/35 و 92/35 درصد بود. تبیین یافتهها حاکی از این بود که زوجدرمانی متمرکز بر هیجان نسبت به زوج درمانی رفتاری یکپارچهنگر از نظر اندازه تغییر، کلیت و ثبات ارجحیت دارد.

اثربخشی زوجدرمانی یکپارچه نگر به روش دایمند بر هماهنگی زناشویی و شایستگی والدگری پس از زایش نخستین فرزند
پژوهشهای اخیر نشانگر کاهش سلامت روانی زوجین پس از زایش فرزند نخست است. بااینوجود، سلامت روانی زوجین همبستگی مثبت معنیداری با گرایش زوجین به فرزند بیشتر داشتن و زایش فرزند پسازآن دارد. هدف پژوهش حاضر بررسی اثربخشی زوجدرمانی یکپارچه نگر به روش دایمند بر هماهنگی زناشویی و شایستگی والدگری پس از زایش نخستین فرزند زوجین شهرستان اهواز در سال 1397 بود. بر این اساس، سه زوج که دارای تکفرزند کمتر از فرزند سهساله بودند، به روش نمونهگیری هدفمند انتخاب شدند. در این پژوهش از طرح آزمایشی تک موردی از نوع خط پایه چندگانه استفاده شد. زوجدرمانی یکپارچه نگر به روش دایمند در سه مرحله خط پایه، مداخله و پیگیری انجام شد و زوجین به سنجه هماهنگی زناشویی و سنجه شایستگی والدگری پاسخ دادند. دادهها به روش ترسیم دیداری، شناسه تغییر پایا (RCI) و فرمول درصد بهبود تحلیل شدند. زوجدرمانی یکپارچه نگر به روش دایمند مایۀ افزایش معنیدار هماهنگی زناشویی (48/26 درصد بهبود) و احساس شایستگی والدگری (02/30 درصد بهبود) زوجین شد.برآیندهای این پژوهش نشان داد زوجدرمانی یکپارچه نگر به روش دایمند میتواند برای انتقال به مرحله والدگری کارساز باشد و خانواده درمانگران میتوانند از این روش درمانی برای بهبود روابط زناشویی زوجین ایرانی استفاده کنند.

بازنمایی عوامل فردی مؤثر در احتمال آسیب طلاق: یک بررسی پدیدارشناسی
افزایش احتمال بروز طلاق از شومترین واقعیتها و مخاطرهآمیزترین پدیدههایی است که میتواند پویایی و سلامت هر جامعهای را با تهدیدهای جدی همراه نماید، بدین منظور پژوهش حاضر با هدف شناسایی و بازنمایی شناسههای فردی اثرگذار در احتمال آسیب طلاق انجامشده است که با رویکرد کیفی و به روش پدیدارشناسی از نوع توصیفی انجامشده است. میدان پژوهش دربرگیرندۀ همه زوجهای در شرف ازدواج مراجعهکننده به مرکز بهداشت استان خراسان رضوی و همه درخواستکنندگان طلاق مراجعهکننده به مراکز مداخلۀ بهزیستی است. شرکتکنندگان دربرگیرنده 15 زوج در شرف ازدواج، 20 زوج درخواستکننده طلاق که با روش نمونهگیری هدفمند از نوع ملاکی انتخاب شدند. ملاکهای ورود به این پژوهش در درخواستکنندگان ازدواج عبارت بود از: سن میان 18 سال تا 40 سال، تحصیلات سوم راهنمایی به بالا، ازدواج نخست و تمایل به مشارکت در پژوهش؛ در افراد درخواستکننده طلاق، افزون بر موارد فوق مدت زندگی زناشویی حداکثر 10 سال نیز لحاظ گردید. از فن مصاحبۀ، عمیق و نیمه ساختاریافته بهمنظور گردآوری دادهها استفاده شد. مصاحبهها در یک دورۀ ششماهه جمعآوری، ثبت و با بهکارگیری راهبرد هفت مرحلهای کلایزی تحلیل شد که سرانجام به شناسایی 579 کد اولیه، 25 مقولۀ فرعی و هفت مقولۀ اصلی اثرگذار در افزایش احتمال آسیب طلاق دربرگیرنده: خلقوخو، بلوغ فکری و روانی، مهارتهای فردی، باورهای فردی، شیوه زندگی، مسائل شخصیتی و ویژگی ظاهری منتج گردید.

پیشبینی آمادگی برای وابستگی به مصرف مواد روانگردان بر پایه کارکردهای خانواده و طرحوارههای ناسازگار نخستین
با توجه به نقش عوامل فردی و خانوادگی در وابستگی به مصرف مواد روانگردان، پژوهش حاضر با هدف بررسی نقش طرحوارههای ناسازگار نخستین و کارکردهای خانواده در آمادگی برای وابستگی به مصرف مواد روانگردان انجام شد. جامعۀ آماری دربرگیرنده دانشجویان دانشگاه آزاد اسلامی، واحد قم در سال تحصیلی 95-96 که 12050 نفر بود و از میان آنها با بهکارگیری فرمول کوکران 300 نفر به شکل نمونهگیری خوشهای چندمرحلهای انتخاب شدند. ابزارهای مورداستفاده دربرگیرنده پیشبینی گرایش به اعتیاد (وید و همکاران،1992)، کارکردهای خانواده (کیتنرو همکاران،1996)و طرحوارههای ناسازگار نخستین (یانگ و همکاران،1995) بودند. تحلیل دادههای پژوهش با بهکارگیری روش همبستگی پیرسون، آزمون تی گروههای مستقل، تحلیل واریانس یکطرفه و رگرسیون چندگانه انجام گرفت. برآیندها نشان دادند که مقیاس اصلی آمادگی برای وابستگی به مصرف مواد روانگردان با طرحوارههای رهاشدگی/ بیثباتی (141/0r=، P<./01)، وابستگی/ بیکفایتی (143/0 r=،P<./01) و آسیبپذیری در برابر زیان و بیماری (173/0r=،، P<./01) همبستگی معنیدار دارد. کارکردهای خانواده تنها با مقیاس اعتراف به اعتیاد همبستگی معنیدار داشت (178/0 r=، P<./01) و با مقیاس اعتراف به اعتیاد و الکلیسم همبستگی معنیداری ندارد. طرحواره آسیبپذیری در برابر زیان و بیماری و طرحواره ایثارگری تنها 5 درصد تغییرات آمادگی برای وابستگی به مصرف مواد روانگردان را تبیین کردند. با توجه به یافتههای پژوهش به مشاوران و روان شناسان پیشنهاد میشود هر دو متغیر کارکردهای خانواده و طرحوارههای ناسازگار نخستین در ارتباط با پیشگیری در وابستگی به مصرف مواد روانگردان موردتوجه قرار دهند.

هم سنجی تأثیر آموزش مبتنی بر پذیرش و تعهد و آموزش بهبود مهارتهای رویارویی زوجی بر بدریختی انگاری بدن و بهزیستی ذهنی زنان دچار سرطان پستان
سرطان پستان و جراحی آن میتواند وضعیتهای روانشناختی متعارضی را به دنبال داشته باشد. هدف پژوهش هم سنجی اثربخشی آموزش مبتنی بر پذیرش و تعهد و آموزش بهبود مهارتهای رویارویی زوجی بر بدریختی انگاری بدن و بهزیستی ذهنی زنان دچار سرطان پستان بود. این پژوهش یک طرح نیمه آزمایشی با پیشآزمون، پسآزمون و گروه گواه و پیگیری 3 ماهه بود. جامعه پژوهش بیماران زن دچار سرطان پستان مراجعهکننده به بیمارستان سیدالشهدا اصفهان در سال 1396 بود. از میان آنها 45 نفر از داوطلبین به روش نمونهگیری هدفمند پس از بررسی ملاکهای ورود انتخاب و بهصورت تصادفی در گروههای آزمایش و گواه گمارش شدند. مداخلات طی 9 نشست 90 دقیقهای اجرا شد، گروه گواه در لیست انتظار قرار گرفت. دادهها با ابزارهای نگرانی از تصویر بدن لیتلتون و همکاران (2005) و بهزیستی ذهنی کییز و ماگیارمو (2003) گردآوری شد. برآیندهای تحلیل واریانس اندازههای مکرر نشان داد که میان گروههای آزمایش با گروه گواه در بدریختی انگاری بدن و بهزیستی ذهنی تفاوت معنیداری وجود دارد و آموزش مبتنی بر پذیرش و تعهد اثرات بیشتری را در سنجش با آموزش مهارتهای رویارویی زوجی بر بهزیستی ذهنی داشته است (05/0p<). بر پایه یافتهها، آموزش مبتنی بر پذیرش و تعهد موجب فراگیری راهکارهای کارآمد در مدیریت هیجانات، احساسات منفی و عمل متعهدانه شده و آموزش مهارتهای رویارویی زوجی میتواند منجر به بالا رفتن توانمندی در مدیریت فشار روانی برآمده از بیماری شود. مداخلاتی که در کنار کاهش علائم بدریختی انگاری بدن، میتوانند بهزیستی ذهنی زنان دچار سرطان پستان را ارتقا دهند.

هم سنجی اثربخشی رویکرد خانوادهدرمانی بوئن و مدل مک مستر بر بیآلایشی، تنظیم شناختی هیجان و کارکرد خانواده در زوجین درخواستکننده طلاق
پژوهش حاضر با هدف هم سنجی اثربخشی خانوادهدرمانی بوئن و مک مستر بر بیآلایشی، تنظیم شناختی هیجان و کارکرد خانواده در زوجین درخواستکننده طلاق صورت گرفت. روش پژوهش، نیمه آزمایشی به شیوه پیشآزمون- پسآزمون با گروه گواه و پیگیری سهماهه بود. جامعه آماری دربرگیرنده همه زوجین درخواستکننده طلاق ارجاع دادهشده از سوی دادگاه خانواده به مراکز مشاوره شهر رشت در سال 1396 بودند. از این جامعه 48 نفر از زوجین با داشتن ملاکهای ورود به پژوهش، به شیوه نمونهگیری در دسترس انتخاب شدند و با گمارش تصادفی در سه گروه 16 نفره قرار گرفتند؛ ولی به دلیل افت آزمودنیها، تعداد نفرات هر گروه به 10 نفر رسید. دو رویکرد خانوادهدرمانی بوئن و مکمستر در گروههای آزمایش به مدت 8 نشست 2 ساعته اجرا شد و گروه گواه، درمانی دریافت نکرد. پس از پایان نشستها، از هر سه گروه پسآزمون و پیگیری سهماهه گرفته شد. ابزارهای مورداستفاده مقیاس بیآلایشی واکر و تامپسون، تنظیم هیجان شناختی گارنفسکی و سنجش کارکرد خانواده اپشتاین بودند. برآیندهای تحلیل کوواریانس چندمتغیری نشان داد خانوادهدرمانی بوئن و مکمستر بر بیآلایشی، تنظیم شناختی هیجان و کارکرد خانواده تأثیر معنیداری داشته است (001/0>P). همچنین، برآیندهای آزمون تعقیبی نشان داد میان اثربخشی رویکرد بوئن و مک مستر، بر بیآلایشی، تنظیم شناختی هیجان و کارکرد خانواده تفاوت معنیداری وجود ندارد. با توجه به یافتههای پژوهش به مشاوران و روانشناسان پیشنهاد میشود متغیرهای بیآلایشی، تنظیم شناختی هیجان و کارکرد خانواده را در کار با زوجین درخواستکننده طلاق موردتوجه قرار دهند.

نقش میانجی ابعاد مراقبت در رابطه میان ابعاد دلبستگی و رضایت جنسی زوجین در چرخههای زندگی خانواده
یکی از تکالیف تحولی باارزش در دوره بزرگسالی، برقراری و حفظ روابط صمیمانه است، که بهوسیله سه نظام دلبستگی، مراقبت و جنسی هدایت میشوند. پژوهش حاضر با هدف بررسی نقش میانجی ابعاد مراقبت در رابطه میان ابعاد دلبستگی و رضایت جنسی زوجین در چرخههای زندگی خانواده انجام شد. روش پژوهش توصیفی از نوع همبستگی و جامعه آماری دربرگیرنده زوجهای ساکن تهران در سال 96-1395 بود؛ که دستکم یکی از آنها تحصیلات دانشگاهی داشته باشند. تعداد ۲۴۴ زوج (488 نفر) به روش نمونهگیری هدفمند و گلوله برفی از دانشگاه شهید بهشتی و خوابگاههای متأهلی دانشگاههای شهید بهشتی، امام صادق (ع)، تربیت مدرس و امیرکبیر انتخاب و ﭘﺮﺳﺸﻨﺎﻣﻪ رﺿﺎﯾﺖ ﺟﻨﺴﯽ (ﻻرﺳـﻮن، و همکاران 1998)، پرسشنامه مراقبت (کانس و شاور، 1994)، و پرسشنامه مقیاسهای رابطه (گریفین و بارثولومیو، 1994) را تکمیل کردند. دادهها با بهکارگیری نرخ همبستگی پیرسون و تحلیل مسیر تحلیل شد. یافتهها نشان داد روابط معنیدار (P<0.01) متفاوتی میان این متغیرها در چرخههای زندگی خانواده، دربرگیرنده آشنایی، زوج بدون فرزند و خانواده دارای کودک، دیده میشود. یافتهها همچنین حاکی از بیشترین تعداد روابط معنیدار (P<0.01) در چرخه زوج بدون فرزند بود. بنابراین متغیر چرخههای زندگی خانواده میتواند رابطه میان این سه نظام را تحت تأثیر قرار دهد، و بهوسیله آن بخشی از ناهمسویی پژوهشهای پیشین در زمینه رابطه میان این سه نظام تبیین میشود. بنابراین ضروری است زوج درمانگران در طراحی مداخلات خود بهمنظور بهبود رضایتمندی جنسی، مراقبت مؤثر، و دلبستگی ایمن میان زوجها، مرحله کنونی چرخه زندگی خانواده را در نظر گیرند.
